For more than sixty years, we have known that women with PCOS are prone to infrequent periods and infertility. When a section of the ovaries was removed for microscopic examination, many women with PCOS became pregnant. The exact mode of action has not yet been fully determined. Those with expertise in minimally invasive surgery have shown that small holes can be drilled into polycystic ovaries at the time of laparoscopy with clinical benefit. Only time will determine whether metformin
(12) or ovarian drilling will prove to be more effective but it would seem prudent to try a medical treatment first.
